EXIT Program Intake
Step One – Complete Referral Form
To begin the intake process, please complete our Referral Form. You can complete it using the form below or by downloading our fillable PDF and emailing it to intake@resetcalgary.ca or by fax to 403-205-5549.

You can complete the form yourself (self-referral), or with a referring agency. You do not need to be referred by an agency to be eligible for the EXIT Program.

Step Two – Review
Once you have completed and submitted the form, we will review the information that you provided. During review we assess eligibility, safety and current supports, and triage appropriately.

If you submit a Referral Form over the weekend, please allow us 48-72 hours for review.

We will contact you using your preferred contact method as indicated on your Referral Form to schedule an intake call.

Step Three – Intake Call
During this call we will go over your Referral Form with you and provide information about the EXIT Program and what to expect. We will plan for how we can best support you and if it is determined that the EXIT Program is a good fit for you, you will be provided a bed date.
Step Four – Waitlist & Check Ins

Once you are given a bed date, you are put on the waitlist. The waitlist prioritizes those who are in urgent need for safety, and timelines can change. You will be given a time estimate for the waitlist, but your bed date may move up or down.

While on the waitlist, you are expected to do check ins over the phone with Intake twice a week. These check ins are to maintain connection, safety, provide support in the community until your bed date arrives, and confirm that you are still interested in coming to the EXIT Program.

 If you do not check in with us, we will attempt to contact you, but if we do not make contact, you may lose your bed date.
